Self Pay Clients – Not Billing Insurance

 

The Standard Fees:

55 minute follow up sessions $125 – length of typical session

55 minute Initial session is $150.00

 

Subsidized Fee Based on Annual Income:

55 minute follow up sessions range from $95-125

55 minute initial session ranges from $110-150

 

Fees for longer or shorter sessions are adjusted accordingly for the standard fee as well as subsidized fees.

 

What is a Subsidized Fee?

~ Fee Subsidy

In order to make therapy accessible to a greater number of people, a Fee Subsidy or Sliding Scale is offered to all self pay clients who are not billing insurance.  The fee is based on total family annual income before taxes.  On this sliding scale everyone pays a similar percentage of their income for the same services.  Some may not feel comfortable sharing this information and therefore no one is obligated to use the sliding scale, but everyone is offered the sliding scale.  If you do not choose to take advantage of the sliding scale, your fee for services will be the standard rate.  Fee for services may be changed during the course of therapy for 2 reasons: 1. a client’s total gross income increases or decreases.  Clients are expected to inform therapist of such changes promptly.  2. Therapist retains the right to change a fee agreement for all current and future clients at any time.

 

Client is to fill out the Sliding Scale Worksheet to determine fee for services.  When form is complete, Client will return form to Therapist.  In order to ensure all clients are treated equally, a copy of the form will be retained in Client’s confidential records.  If Client would like help in filling out the Sliding Scale Worksheet, Therapist will be happy to assist.

 

~ Limitations of Subsidy

If your annual income is $75,000.00 or less you may choose to pay the lowest fee of $95 for a 55 minute session.  If you annual income ranges between 75Kand 100K your fee will range from $95-$125.  In order to receive the subsidy, you will be required to fill out the Sliding Scale Worksheet. 

If you know your yearly income is equal to or greater than $100,000.00, then you will be paying the standard fee of $100.  In this case you are not required to fill out the Sliding Scale Worksheet.
